Before you click on the link.
1. When the doc opens you will not be able to edit it.
2. Under File> Email> Email this file:
Enter your Email Address (Send yourself a copy won’t work without a “To” address)
Click “Send”
Open the attachment, edit or complete the form, and Email it to the proper committee.
You may find the Web site a practical way to send this Email.
Note: PDF documents (Created in PDF format) are only editable by PDF editor, MS Word, Libre Office Draw, and Google Docs. All edits should be saved as PDF (Google Docs will save as PDF for you)
